Founded in 2014, the Needs Foundation (NF) is a Puerto Rico (PR) based non-profit organization dedicated to the empowerment of communities with high social needs through community centered care services, education, and support. To this end, NF seeks to establish its roots within PR impoverished communities becoming a catalyst of healthy empowerment by fostering inclusiveness, social equity, and resilience. With a strong community presence, NF has experience working alongside community leaders while designing programs focused on identifying populations with complex chronic health conditions and high psychosocial needs. Owing to our community-based approach, we have been drivers of multiagency concerted effort aimed at identifying social and physical determinants of poor community resiliency. NF has a community presence providing leadership, socially just services that allow each community to maintain better overall health and quality of life, and develop healthy, empowered, and self-sustaining communities. Moreover, our experience includes assisting in the coordination and identification of community resources, the empowerment and identification of community leaders as well as the gathering of data from each individual and communities to evaluate socioeconomic and health conditions. NF has been collecting data from the past 4 years serving underserved communities in Puerto Rico. Our data demonstrates demographics, literacy levels, living conditions, health assessments, geographical “Hotspotting”, generational trends, service inefficiencies, and other variations of stratified information from every community served in PR for the past 4 years.
