Optimus Health Care, formerly Bridgeport Community Health Center, Inc., is the largest provider of primary health care services in Southwestern CT. With 12 service delivery sites located in the Cities of Bridgeport and Stamford, and the Town of Stratford, we provide comprehensive health care services to everyone, regardless of race, income and insurance status. We will provide you and your family the best quality of care!We are a private non-profit organization operating 12 JCAHO accredited community health centers, including health care centers for the homeless, a walk-in primary care center, dental clinics, and general family health practice.Mission: To improve the overall health of our communities in Bridgeport, Stamford, and Stratford, particularly the medically underserved, by providing preventive, primary care and supplemental health care services along with health education in a culturally sensitive manner regardless of ones ability to pay.Business Goals: (2004 - 2010) 1. Optimus Health Care Inc. will improve access to affordable, accessible, and quality preventive, primary health care, oral health, behavioral health services and ancillary services for the uninsured, underserved and medically indigent populations in Bridgeport, Stratford, and Stamford, CT. 2. Optimus Health Care Inc. will improve the agency's performance in its administrative, clinical, and fiscal systems to ensure its primary health care and supportive services are delivered in a safe, appropriate, efficient, effective, of quality, and patient-centered manner in Bridgeport, Stratford, and Stamford, CT. 3. Optimus Health Care Inc. will sustain and expand the agency's structure and capacity to serve the uninsured, underserved and medically indigent communities in Bridgeport, Stratford, and Stamford, CT. 4. Optimus Health Care Inc. will improve its Staff Performance Systems. Program Goals: (2004 - 2010)Contribute in the reduction of low birth weight infants among minority populations in Bridgeport, Stamford and Stratford by offering and providing perinatal care services to women within OPTHC 's service areas.Contribute to the reduction of number of births to teenage mothers in Bridgeport, Stamford, and Stratford by providing family planning services for teenagers in BCHC's service areas.Contribute in the increase of proportion of children who completed basic primary immunization series by age two in Bridgeport, Stamford and Stratford by continuing to provide primary health services, including immunization, to children within OPTHC's service areas from 2/1/2005 to 1/31/2010. Contribute to the reduction of asthma hospitalization rates among minority children in Bridgeport and Stamford by providing clinical management and community-based health education on prevention and control of micro-environmental asthma triggers from 2/1/2004 to 1/31/2010. Contribute in the reduction of the proportion of minority children and adolescents in Bridgeport, Stamford and Stratford who are obese and overweight by providing a community-based child-centered weight management program and local community actions to promote increase physical activity and healthy eating. Contribute to the reduction of mortality and morbidity due to unintentional injuries among children and adolescents in Bridgeport, Stamford, and Stratford by continuing to provide well-child services that includes health education and counseling on child safety, growth and development, screening for risk factors including domestic violence from 2/1/2005 to 1/31/2010.Contribute in the reduction of incidence rate of STDs among minority adolescents in Bridgeport, Stamford and Stratford by providing culturally appropriate and non-judgmental adolescent sexuality education including promotion of delaying initiation of sexual intercourse, screening for STDs, clinical management, and preventive services including distribution of condoms from 2/1/2005 to 1/31/2010. Contribute to the reduction of CVD morbidity and mortality among adult minority populations in Bridgeport, Stamford and Stratford by providing access to primary health care services that include CVD screening, risk factor assessment, prevention education and other clinical intervention measures.Contribute to the CT Department of Public Health priorities of improving rates of cancer screening and reducing proportion of the population with moderately modifiable risk factors to cancer by providing access to primary health care services that includes cancer screening, risk factor assessment, prevention education and other clinical intervention measures to adult minority populations in Bridgeport, Stamford, and Stratford from 2/1/2005 to 1/31/2010.Contribute to the reduction of diabetes complications and hospitalization rates among African-Americans and Hispanics in Bridgeport, Stamford and Stratford by providing access to primary health care services that includes diabetes screening, monitoring and control, and prevention and management of risk factors from 2/1/2005 to 1/31/2010.Contribute to the reduction of injury-related death rates, including deaths from motor vehicle crash, among African-American and Hispanic adult population by providing access to primary health care services that include screening for substance abuse and domestic violence, prevention and referral for treatment of substance abuse, behavior modification strategies, and health education and counseling on safety. Contribute to the control and prevention of TB infection in Bridgeport, Stamford and Stratford by providing TB screening and treatment to immigrant populations from 2/1/2005 to 1/31/2010. Contribute to elimination of barriers to health care services for immigrant population by ensuring delivery of multi-lingual, culturally appropriate, low-cost primary health care services that includes outreach services and information dissemination on available resources for immigrants in Bridgeport, Stamford and Stratford from 2/1/2005 to 1/31/2010.Provide access to primary health care services for the homeless population in Stamford by developing a system of health care delivery with Homeless Shelters and other local agencies providing services to the homeless.Contribute to elimination of barriers to primary health care services for public housing residents by providing outreach, on-site health screenings and referral services in public housing complexes within OPTHC's service areas in Stamford and Bridgeport from 2/1/2005 to 1/31/2010.Manage geriatric health by re-enforcing self-esteem and responsibility at all BCHC's site at Bridgeport, Stratford and Stamford from 2/1/2005-1/31/2010.Contribute to the control, prevention and management of HIV/AIDS problem by providing risk assessment, HIV counseling and testing, and comprehensive health and social services for HIV + patients in Bridgeport from 2/1/2005 to 1/31/2010.
